Responsibilities
- Provide skilled nursing care and treatments to geriatric patients in the community.
- Develop patient-centered care plans and contribute to discharge planning.
- Work in New Castle County, DE, with day or night shift options, one weekend a month, and 2 holidays annually.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
