Job Description
We are looking for experienced Licensed Practical Nurses (LPNs) to join ChristianaCare HomeHealth. As a member of our team, you will provide skilled nursing care to geriatric patients in the community, implement care plans, and contribute to patient-centered approaches. Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.
