Job Description
We are looking for experienced Home Care LPNs to join our team at ChristianaCare. As an LPN in our HomeHealth division, you will provide skilled nursing care to geriatric patients in the community.
Responsibilities
- Give skilled nursing care and prescribed treatment to geriatric patients in the community.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Must possess a current DE LPN license or compact state license.
