Job Description
We are looking for an experienced Home Care LPN to join ChristianaCare's HomeHealth team. If you are a Licensed Practical Nurse with expertise in providing skilled nursing care to geriatric patients, this is an opportunity to excel in your career. You will work in New Castle County, DE, on a full-time, part-time, or per diem basis, supporting patient care and discharge planning.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Implement patient-centered care plans in alignment with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.
Job Types

Compensation
Pay: $22.00 - $28.00 per hour
Expected hours: 20 – 40 per week
