Job Description
We are looking for an experienced Home Care LPN to join our team at ChristianaCare. As a Full-Time, Part-Time, or Per Diem LPN, you will provide skilled nursing care to geriatric patients in the community, implement care plans, and participate in discharge planning. You will have the opportunity to support New Castle County, DE, working day or night shifts, one weekend a month, and 2 holidays per year.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
- Implement goal-oriented and patient-centered care plans, consistent with available resources.
- Participate in developing discharge plans for patients.
Requirements
- HSD or GED required.
- Must possess a current DE LPN license or compact state license.

$22.00 - $28.00 per hour
