Job Description
We are seeking compassionate and dedicated Home Care LPNs to join our team at ChristianaCare. As an LPN, you will provide skilled nursing care to geriatric patients in the community, implement care plans, and contribute to patient discharge planning. This position offers the opportunity to work in New Castle County, DE, with scheduling options including full-time, part-time, and per diem roles, with competitive compensation based on experience.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Implement patient-centered care plans and discharge planning.
- Participate in goal-oriented care delivery consistent with available resources.
Requirements
- High School Diploma (HSD) or GED required.
- Current DE LPN license or compact state license.
