Job Description

We are looking for a dedicated Home Care LPN to join our team at ChristianaCare. As a Licensed Practical Nurse, you will provide skilled nursing care to geriatric patients in the community, implementing patient-centered care plans and supporting discharge planning.

Responsibilities

• Deliver skilled nursing care and prescribed treatments to geriatric patients.
• Implement patient-centered care plans aligned with available resources.
• Participate in discharge planning for patients.

Requirements

• High School Diploma (HSD) or equivalent required.
• Current DE LPN license or compact state license.
• Available for full-time, part-time, or per diem roles.

• Competitive hourly pay: $22.00 - $28.00
