Job Description
ChristianaCare HomeHealth is looking for a dedicated Home Care LPN to join our team. As a Full Time, Part Time, or Per Diem LPN, you will provide skilled nursing care to geriatric patients in the community, implement care plans, and participate in the discharge planning process. Join us in supporting New Castle County, DE, with day or night shifts, one weekend per month, and 2 holidays per year.
Responsibilities
- Provide skilled nursing care and prescribed treatments to geriatric patients.
- Implement patient-centered care plans in line with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
