Job Description
We are seeking Home Care LPNs to join ChristianaCare's HomeHealth team. This is a rewarding opportunity to provide skilled nursing care to geriatric patients in the community. Successful candidates will implement care plans, participate in discharge planning, and support patients with goal-oriented and patient-centered treatment.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Current DE LPN license, or compact state license.
Benefits
- Competitive pay: $22.00 - $28.00 per hour.
- Flexible schedule: 20 – 40 hours per week.
