ChristianaCare HomeHealth is seeking a dedicated and experienced Home Care LPN to join our team. As a Licensed Practical Nurse, you will provide skilled nursing care to geriatric patients in the community, following care plans to ensure patient well-being and recovery.
Responsibilities
- Administer skilled nursing care and treatments to geriatric patients.
- Implement patient-centered care plans with a focus on patient goals.
- Participate in discharge planning for patients.
Requirements
- High School Diploma or GED.
- Current DE LPN license or compact state license.
