Job Description
We are looking for an experienced Home Care LPN to join our team at ChristianaCare. As a part of our HomeHealth division, you will provide skilled nursing care and treatment to geriatric patients in the community. Responsibilities
- Deliver skilled nursing care and treatment to geriatric patients.
- Implement patient-centered care plans in line with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
