Job Description
We are looking for a skilled Home Care LPN to join our team at ChristianaCare. As a Licensed Practical Nurse in home health, you will provide quality care to geriatric patients in the community, implementing care plans and ensuring patient well-being.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in a home health setting.
- Develop and implement patient-centered care plans in line with available resources.

Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
