Job Description
We are looking for experienced Licensed Practical Nurses (LPNs) to join our Home Health team at ChristianaCare. You will have the opportunity to work with geriatric patients in the community and play a vital role in providing skilled nursing care. The position offers a competitive salary ranging from $22 to $28 per hour based on experience. Responsibilities
- Provide skilled nursing care and treatments to geriatric patients in the community.
- Implement goal-oriented and patient-centered care plans.
- Participate in discharge planning for patients.

- Current DE LPN license or compact state license.
