Job Description
We are looking for a dedicated Licensed Practical Nurse (LPN) to join our ChristianaCare HomeHealth team. As an LPN, you will provide skilled nursing care to geriatric patients in the community, implement care plans, and participate in discharge planning. This role offers the opportunity to work full-time, part-time, or per diem, supporting patients in New Castle County, DE.
Responsibilities
- Provide skilled nursing care and treatments to geriatric patients.
- Implement goal-oriented, patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Current DE LPN license or compact state license.
