Job Description
We are looking for experienced Licensed Practical Nurses (LPNs) to join our Home Care team at ChristianaCare. If you are a dedicated LPN with a passion for providing skilled nursing care to geriatric patients in the community, we want you to be a part of our innovative and supportive environment.
Responsibilities
- Provide skilled nursing care and treatments to geriatric patients in a community setting.
- Implement patient-centered care plans in accordance with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
