Job Description
We are looking for experienced Licensed Practical Nurses (LPNs) to join our HomeHealth team at ChristianaCare. As an LPN in our Home Care division, you will provide skilled nursing care to geriatric patients in the community and support goal-oriented care plans. If you are dedicated to patient-centered care and have a current DE LPN license or compact state license, we invite you to apply.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
