Job Description
We are seeking experienced Licensed Practical Nurses (LPNs) to join our Home Care team at ChristianaCare. As an LPN, you will be responsible for delivering skilled nursing care and treatments to geriatric patients within the community. Responsibilities
- Provide skilled nursing care and treatments to geriatric patients in the community.
- Implement patient-centered care plans in alignment with available resources.
- Participate in discharge planning for patients.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.
