Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
- Implement goal-oriented, patient-centered care plans consistent with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High school diploma or GED required.
- Must possess a current Delaware LPN license or compact state license.
