Job Description
We are looking for a dedicated Licensed Practical Nurse (LPN) to join ChristianaCare's HomeHealth team. As an LPN, you will provide skilled nursing care and treatment to geriatric patients in the community, implementing patient-centered care plans and participating in discharge planning.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Must have a current DE LPN license or compact state license.
Job Types

Pay
$22.00 - $28.00 per hour
