Responsibilities
- Provide skilled nursing care and treatments to geriatric patients in the community.
- Implement goal-oriented care plans tailored to patients' needs.
- Participate in discharge planning processes.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
