Job Description

Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Implement and adapt care plans to meet patients' goals and resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Current DE LPN license or compact state license.
