Job Description
We are looking for experienced Licensed Practical Nurses (LPNs) to join our ChristianaCare HomeHealth team. If you are dedicated to providing skilled nursing care to geriatric patients in the community, we offer a supportive environment where you can excel in your career. As part of our team, you will implement patient-centered care plans and contribute to the discharge planning process.
Responsibilities
- Provide skilled nursing care and prescribed treatments to geriatric patients.
- Implement goal-oriented care plans that are consistent with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Must possess a current DE LPN license or compact state license.
Details
- Job Types: Per diem, Part-time, Full-time
- Pay: $22.00 - $28.00 per hour
- Expected hours: 20 – 40 per week
