Licensed Practical Nurse MDS Coordinator

Job Description

Genesis Health is seeking a highly skilled Licensed Practical Nurse (LPN) MDS Coordinator to join our team. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing essential items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure appropriate patient care and nursing center reimbursement.

Position Highlights

• Direct the interdisciplinary RAI process in compliance with guidelines.
• Develop individualized resident Care Plans based on Federal and State regulations.
• Train new team members on clinical content and MDS completion.

Requirements

• LPN license in good standing.
• Prior experience in a healthcare facility.
• Knowledge of nursing practices, medical procedures, and long-term care regulations.
• Computer proficiency with Windows applications.
• AccuMed experience preferred.
