Job Description

At Aveanna Healthcare, we are seeking a dedicated Licensed Practical Nurse (LPN) to join our team. The LPN will be responsible for providing skilled nursing care, following care plans, and ensuring patient well-being under the supervision of a Registered Nurse. This role requires attention to detail, effective communication, and a commitment to excellence in patient care.

Responsibilities

  • Deliver and coordinate quality patient care in accordance with physician orders.
  • Observe and assess patient condition, report changes to supervisors and physicians.
  • Document all activities, assessments, and care coordination promptly.
  • Participate in the development and updates of nursing care plans.
  • Take appropriate nursing actions based on assessments and achieve expected outcomes.
  • Educate patients and families on self-care techniques, disease management, and prevention strategies.

Requirements

  • Graduate of an accredited school of nursing.
  • Current, unrestricted state license as a Licensed Nurse.
  • Current CPR certification.
  • Proficiency in clinical assessments, documentation, and compliance with nursing care standards.
  • Additional state-specific requirements may apply.

Preferences

  • Six months of recent experience in a clinical care setting.
  • Home health experience is advantageous.

Physical Requirements

  • Must be able to speak, write, read, understand English.
  • Ability to travel and lift up to 50 pounds.

  • Possible exposure to blood, bodily fluids, and infectious diseases.
