Responsibilities

• Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
• Implement patient-centered care plans consistent with available resources.
• Participate in developing discharge plans for patients.

Requirements

• High School Diploma or GED required.
• Must possess a current DE LPN license or compact state license.
