Job Description
We are currently seeking dedicated and experienced LPN Home Health Nurses to join our team at ChristianaCare. If you are a Licensed Practical Nurse with a passion for providing quality care to patients in their homes, this is an excellent opportunity for you.
Duties and Responsibilities
- Provide skilled nursing care and treatments to geriatric patients in the community.
- Implement patient-centered care plans in alignment with available resources.
- Participate in discharge planning for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
- Available for Full-Time, Part-Time, Per Diem job types with flexible hours.
