ChristianaCare HomeHealth is seeking Full Time, Part Time, and Per Diem LPN's to support New Castle County, DE. The successful candidates will provide skilled nursing care and prescribed treatment to geriatric patients in the community.
Duties and Responsibilities:
- Give skilled nursing care and prescribed treatment to geriatric patients in the community.
- Implement care plans that are goal-oriented, patient-centered, and consistent with available resources.
- Participate in developing discharge plans for patients.
Requirements:
- HSD or GED required.
- Must possess a current DE LPN license or compact state license.
