Job Description
We are seeking dedicated Licensed Practical Nurses (LPNs) to join our Home Health team at ChristianaCare. As an LPN in our HomeHealth division, you will provide skilled nursing care to geriatric patients in the community, implementing care plans and participating in discharge planning.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients.
- Implement patient-centered care plans in alignment with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.

$22.00 - $28.00 per hour
