Job Description
We are looking for skilled Home Care Licensed Practical Nurses (LPNs) to join our team at ChristianaCare HomeHealth. As a part of our dedicated team, you will provide exceptional care to geriatric patients in the community, implementing personalized treatment plans and supporting patient discharge planning.
Responsibilities
- Deliver skilled nursing care and treatments to geriatric patients.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing patient discharge plans.
Requirements
- High School Diploma (HSD) or GED required.
- Current DE LPN license or compact state license.
