Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Develop and implement goal-oriented and patient-centered care plans.
- Participate in discharge planning for patients.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.
- Available for day shift or night shift with one weekend a month and 2 holidays per year.
