Job Description
We are looking for skilled and compassionate Licensed Practical Nurses (LPNs) to join our HomeHealth team at ChristianaCare. As an LPN in our HomeHealth division, you will provide exceptional nursing care to geriatric patients in the community, implement goal-oriented care plans, and contribute to patient discharge planning.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients.
- Implement goal-oriented, patient-centered care plans.
- Participate in patient discharge planning.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.
