Job Description
ChristianaCare HomeHealth is seeking experienced LPNs to join our team in New Castle County, DE. As an LPN in Home Care, you will provide skilled nursing care to geriatric patients, implement care plans, and participate in discharge planning. Full-time, part-time, and per diem positions available with flexible scheduling options.
Responsibilities
- Deliver skilled nursing care and treatments to geriatric patients in the community.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing patient discharge plans.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
- Experience in Home Health or related medical specialties preferred.
