Job Description
We are seeking experienced Licensed Practical Nurses (LPNs) to join our ChristianaCare HomeHealth team. The LPNs will provide skilled nursing care to geriatric patients, implement care plans, and participate in discharge planning.
Responsibilities
- Provide skilled nursing care and administer treatment to geriatric patients in the community.
- Implement patient-centered care plans in line with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.
