MDS Coordinator

Job Description

We are looking for an experienced MDS Coordinator to join The University of Vermont Health Network team. In this role, you will work under the leadership of the Director of Nursing Services to conduct thorough assessments of residents' needs and develop personalized care plans. Collaboration with Nursing Unit Managers is crucial to ensure effective communication of resident's status and changes. Compliance with nursing standards and promoting the goals of our healthcare facility are key responsibilities.

Education & Experience

• Graduation from an accredited Nursing Education Program with current RN licensure or compact licensure recognized by the State of Vermont.
• Minimum 5 years of MDS experience post-2019 is mandatory for this remote position.
• Three to five years of experience in a long-term care setting in leadership roles such as Team Leader or Charge Nurse.
