Minimal Data Set Coordinator - Licensed Practical Nurse

Job Description

Genesis Health is seeking a Licensed Practical Nurse to fill the role of Minimal Data Set Coordinator. The LPN MDS Coordinator plays a pivotal role in the Resident Assessment Process by completing key items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure optimal patient care and nursing center reimbursement.

Position Highlights

• Direct the interdisciplinary RAI process, ensuring compliance with guidelines and electronic transmission of the MDS.
• Develop individualized resident Care Plans in adherence to Federal and State regulations.
• Provide training on clinical content and electronic completion of MDS items to new team members.

Requirements

• Active LPN license.
• Prior experience in hospital or long-term care settings.
• Familiarity with rehabilitative and restorative nursing practices.
• Knowledge of laws and regulations governing long-term care.
• Computer proficiency, preferably with AccuMed.
• Previous experience as an MDS Coordinator is a plus.
• Current BLS/CPR certificate.
