Job Description
Genesis Health is seeking a detail-oriented Minimal Data Set Coordinator who is a Licensed Practical Nurse (LPN) to join our team. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing essential components of the Minimum Data Set (MDS) and related Care Area Assessments (CAAs) to ensure optimal patient care and proper nursing center reimbursement.
Position Highlights
- Lead the interdisciplinary Resident Assessment Instrument (RAI) process, ensuring compliance with completion guidelines and electronic transmission of MDS.
- Assist in the development of personalized resident Care Plans, identifying resident needs and expected outcomes in adherence to Federal and State regulations.
- Provide training to new team members on clinical content and electronic completion of MDS items.
Requirements
- Licensed Practical Nurse (LPN) in good standing with current state license.
- Prior experience in hospitals, long-term care facilities, or healthcare-related settings.
- Proficiency in general, rehabilitative, and restorative nursing practices, along with knowledge of relevant laws and regulations.
- Computer skills including Windows applications; experience with AccuMed is a plus.
- Previous experience as an MDS Coordinator preferred.
- Certification in Basic Life Support/Cardiopulmonary Resuscitation (BLS/CPR) required.
