Nurse-LPN MDS Coordinator

Job Description

Genesis Healthcare is seeking a Nurse-LPN MDS Coordinator to join our team. As a key member of our healthcare facility, you will participate in the Resident Assessment Process by completing items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure optimal patient care and proper nursing center reimbursement.

Position Highlights

• Direct the interdisciplinary RAI process following completion guidelines.
• Develop individualized resident Care Plans in compliance with regulations.
• Train team members on clinical content and electronic MDS completion.

Requirements

• LPN license in good standing.
• Prior experience in healthcare facilities.
• Knowledge of nursing practices and regulations.
• Computer proficiency.
• AccuMed experience preferred.
• Previous MDS Coordinator experience preferred.
• BLS/CPR certification required.
