Job Description
We are looking for a dedicated Overnight Licensed Practical Nurse (LPN) to join our team at Aveanna Healthcare. As an LPN, you will be responsible for delivering high-quality nursing care to our patients, following care plans and physicians' orders. Your role will involve continuous patient assessment, documentation of care, and collaboration with supervisors and physicians to ensure optimal patient outcomes.
Key Responsibilities
- Deliver and coordinate quality patient care in adherence to physician orders.
- Monitor and assess patient conditions, reporting changes to supervisors and physicians.
- Document all nursing activities, assessments, and care coordination promptly.
- Implement and update nursing care plans accordingly.
- Recognize and respond to changes in patient needs effectively.
- Adhere to Aveanna policies, state regulations, and nursing best practices.
- Provide patient education on disease management and self-care.
- Maintain nursing competencies through ongoing education and skill evaluations.
Requirements
- Graduate of an accredited nursing school.
- Current, unrestricted LPN license in the state of practice.
- Valid CPR certification.
- Proficiency in clinical assessments and documentation.
- State-specific requirements may apply.
Preferences
- Minimum six months of recent clinical nursing experience.
- Home health experience preferred.

- Detail-oriented with excellent time management skills.
- Effective problem-solving and communication abilities.
- Strong organizational skills.
Physical Requirements
- Ability to speak, write, and understand English.
- Capable of lifting 50 pounds and assisting in patient repositioning.
