Physical Therapist Requirements:

• Graduated from an accredited Physical Therapy (PT) college or university.
• Valid Physical Therapy (PT) license for New York.
• Perform home visit Physical Therapy assessments to evaluate medical conditions and rehabilitation potential.
• Proficiency in EMR documentation preferred.
• CPR Certification required.
