Physical Therapist Requirements:

• Graduated from an accredited Physical Therapy (PT) college or university.
• Unencumbered Physical Therapy (PT) license for New York.
• Conduct home visit Physical Therapy assessments of referred patients.
• Proficient in EMR documentation (experience with Hello Note is beneficial).
• CPR Certification.
