Paradigm Rehab is currently seeking a dedicated Physical Therapist to join our team. As a Physical Therapist, you will play a vital role in helping patients improve their quality of life through personalized therapy services. Physical Therapist Requirements:

• Graduated from an accredited Physical Therapy (PT) college or university.
• Hold an unencumbered Physical Therapy (PT) license for New York.
• Conduct comprehensive home visit assessments and provide direct patient care.
• Proficient in EMR documentation with experience in Hello Note (not required).
• Hold a valid CPR Certification.
