Physical Therapist Requirements:

• Graduated from an accredited Physical Therapy (PT) college or university.
• Must have an unencumbered Physical Therapy (PT) license for New York.
• Conduct home visit Physical Therapy assessments and provide direct patient care.
• Proficient in EMR documentation (experience with Hello Note preferred).
• Must hold CPR Certification.
