Physical Therapist Requirements:

• Graduated from an accredited Physical Therapy college or university.
• Unencumbered Physical Therapy license for New York.
• Conduct home visit assessments, providing direct patient care.
• Proficient in EMR documentation (experience with Hello Note is a plus).
• CPR Certification.
