Physical Therapist Requirements:

• Graduated from an accredited Physical Therapy (PT) program.
• Hold an unencumbered PT license for New York.
• Conduct home visits to assess patients, determine rehabilitation potential, and deliver care.
• Proficient in EMR documentation.
• Hold a valid CPR Certification.
