Physical Therapist Requirements:

• Graduated from an accredited Physical Therapy (PT) college or university.
• Hold an unencumbered Physical Therapy (PT) license for New York.
• Perform home visit Physical Therapy assessments, providing direct patient care and rehabilitation evaluations.
• Proficient in EMR documentation with experience in systems like Hello Note (not required).
• Hold a CPR Certification.
