Responsibilities

• Conduct home visit assessments of patients to evaluate medical conditions and rehabilitation potential.
• Provide direct patient care and document progress accurately using EMR systems.
• Collaborate with patients to establish treatment plans for rehabilitation.
• Ensure compassionate and effective communication with patients and their families.

Requirements

• Graduated from an accredited Physical Therapy program.
• Hold a valid Physical Therapy license for New York.
• Proficiency in EMR documentation; experience with Hello Note is a plus.
• CPR Certification.
