Paradigm Rehab is seeking a dedicated Physical Therapy Specialist to join our compassionate team. As a Physical Therapist, you will play a crucial role in helping individuals improve their quality of life through rehabilitation and care. Responsibilities

• Conduct home visit Physical Therapy assessments for referred patients.
• Provide direct patient care to evaluate medical conditions, functional capabilities, and potential for rehabilitation.
• Proficiently document patient information in our EMR system.
• Maintain CPR Certification.

Requirements

• Graduated from an accredited Physical Therapy (PT) program.
• Hold an unencumbered Physical Therapy (PT) license for New York.
• Experience with EMR documentation preferred.
• Ability to conduct therapy sessions in person.
