• Conduct home visit assessments to evaluate patients' medical conditions and potential for rehabilitation.

• Maintain communication with patients and families to ensure quality care.

Requirements:

• Graduated from an accredited Physical Therapy program.
• Valid and unencumbered Physical Therapy license in New York.
• Proficiency in CPR and EMR systems.
• Ability to work in Brooklyn and conduct in-person visits.
