We are looking for a dedicated Physical Therapy Specialist to join our team at Paradigm Rehab. As a Physical Therapist, you will have the opportunity to make a positive impact on the lives of patients by providing direct care, conducting assessments, and assisting in their rehabilitation journey.

Responsibilities:

• Conduct thorough Physical Therapy assessments in patients' homes.
• Provide direct patient care to evaluate medical conditions, functional capabilities, and rehabilitation potential.
• Document patient information accurately using EMR systems.
• Maintain an unencumbered Physical Therapy license for New York.

Requirements:

• Graduation from an accredited Physical Therapy program.
• Valid Physical Therapy license in New York.
• CPR Certification.
• Familiarity with EMR documentation.
