Physical Therapy Specialist Requirements:

• Graduated from an accredited Physical Therapy (PT) college or university.
• Hold an unencumbered Physical Therapy (PT) license for New York.
• Conduct home visit Physical Therapy assessments focusing on patient care, medical conditions, functional capabilities, and potential for rehabilitation.
• Proficiency in EMR documentation (experience with Hello Note is a plus).
• Current CPR Certification.
